A private of Captain L H Powers Company, [I], of the 9th Iowa Regiment of United States, was enlisted by Capt. Powers, of the ____ Regiment of ___________ at Chickasaw on the 13th day of August 1861, to serve three years; he was born in Cha____? Co. in the Providence of Canada, is thrity nine years of age; five feet 91/2 inches high. light complexion, black eyes, brown hair and by occupation when enlisted a Farmer . During the last two months said soldier has been unfit for duty sixty days.



I certify that I have carefully examined the said William Everingham of Captain Powers Company, and find him incapable of performing the duties of soldier because of:
Diabetes, general Debility, broken down ,Constitution, and general ,unfitness for ,service.
